IMPACT Plus
IMPACT Plus currently has an agreement with Ramey-Estep Homes to provide traditional and innovative behavioral health services to eligible children and their families.
IMPACT Plus is an innovative program based on the Service Team approach. The Service Team stresses the importance of the child’s parent or guardian’s involvement as an equal partner within the team. The Service Team focuses on the unique needs of the child and the services that will help the child cope successfully in their home, school, and community. Other potential team members can include teachers, coaches, therapists, ministers and friends of the family. The child and family are encouraged to include their natural supports as team members. Together, the Service Team assesses the needs of the child and family and develops an individualized service plan. The service plan includes the goals, objectives, and services that are intended to assist in the reduction of the child’s emotional and behavioral symptoms while providing needed support to the child’s family. The services identified in the service plan will include natural supports and community resources in addition to IMPACT Plus services. The team closely monitors and evaluates the implementation of these services for quality and effectiveness, while working with the child and family to enable them to transition to a lower level of care.

Eligibility Citeria
Must be eligible for Medicaid
Must be under the age of 21
Must be at risk of institutional, hospital, or other out of home placement
Must be identified as having a severe behavioral health disorder that has persisted for the past 6 months and is expected to persist for at least six months without intervention
Must be identified as having complex treatment needs
The program is built on the wrap-around perspective. The IMPACT Plus program emphasizes that families are the experts on their own children, and seeks to actively involve parents or guardians in treatment planning and implementation. IMPACT Plus is intended to be a short-term program that empowers children and their families by reducing their need for crisis intervention, developing their ability to function independently, and increasing their effective use of natural and community resources and supports. We strive to build teams for kids who need more support than they are able to receive from traditional out-patient therapy. IMPACT Plus is a collaborative effort among several resources including community based services, mental health, education, health services and the courts at local, regional, and state levels. Ramey-Estep Homes as an IMPACT Plus sub-provider, utilizes this model to coordinate and mobilize services to eligible children.
